Mickey cresce in un quartiere povero di Miami, studia recitazione a New York e si trasferisce a Los Angeles, esordendo in una piccola parte in "1941: allarme a Hollywood".
Appare in "I cancelli del cielo", "Brivido caldo", "Rusty il selvaggio" e "L'anno del dragono".
Ottiene vera fama nell'86 con "Nove settimane e mezzo", film proverbiale per l'erotismo al limite dell'hard.
Recita a fianco di De Niro in "Angel Heart - Ascensore per l'inferno" e nel '91 entra nella boxe professionale.
Nel 1997 è la volta di '"Nove settimane e mezzo 2: la conclusione" e "L'uomo della pioggia" di F. F. Coppola, una delle sue migliori interpretazioni.
Tra il 2000 e il 2002 interpreta film di minor successo tra cui "Animal Factory" (2000) e "C'era una volta in Messico"(2002).
Nel 2004 Mickey Rourke interpreta Eddie e Jordan rispettivamente nei film "Sola nella trappola" e "Man On Fire - il Fuoco della Vendetta". E' tornato in auge con il film "Sin City" (2005) accanto a Bruce Willis, Benicio Del Toro, e ha lavorato nella pellicola "Stormbreaker" (2006).
